Extracurricular activities can take a physical toll on a student's body, from the performance of the musculoskeletal system to injuries that can affect virtually all parts of the body, including the head, heart, joints, bones, and tissues. An annual student physical, conducted by a knowledgeable and experienced health practitioner, is one of the easiest and most effective ways to ensure that a student is physically fit and able to handle the rigors of extracurricular activities, such as athletics, band and cheerleading. Physicals are an important tool in identifying students who may be at risk for health problems.
The UIL Physical Evaluation Medical History Form and Houston Methodist Consent form must be printed, filled out and signed by a parent or guardian prior to the physical. Please bring these signed forms with you on Saturday, May 31.

In the next few months high school and junior high students from the greater Houston area will begin preparing for the 2014-15 school year and their participation in extracurricular activities. Prior to participation in athletic practices, a student athlete must complete a quality health screening to address any potential health risks for their participation.
On Saturday, May 31, 2014, Houston Methodist Hospital will host our annual pre-participation physical event at Reliant (Texans Center) from 8a.m. to 2p.m. We ask that you consider volunteering 3.5 hours of your time by supporting our community initiative with your assistance in a clinical support, general support, or Resident/Fellow/Physician position.
