Extracurricular activities can take a physical toll on a student's body, from the performance of the musculoskeletal system to injuries that can affect virtually all parts of the body, including the head, heart, joints, bones, and tissues. An annual student physical, conducted by a knowledgeable and experienced health practitioner, is one of the easiest and most effective ways to ensure that a student is physically fit and able to handle the rigors of extracurricular activities, such as athletics, band and cheerleading. Physicals are an important tool in identifying students who may be at risk for health problems.

IMPORTANT: The first two pages of the form must be printed, filled out and signed by a parent or guardian prior to the physical. The third page will be filled out and signed by a physician during the physical, so please bring all pages with you to the physical.
